Are we allowed to submit AMVs under our badge name or does it have to be our legal name?

The "Name" field on the form should contain your legal name. If you want your video to be credited differently on the screen and the ballots, there is a separate field "Name for Credits/Ballots" in the section for your video's information where you can use your badge name or studio name.
